- direct or indirect observation by supervising physician, which may include input from other healthcare professionals (nurse, social worker)
- Form 1
- demographic: patient interaction, family interaction, physician interaction, other healthcare professional interaction
- collect 10 observations of achievement (at least 3 clinical observations)
